By Appointment Only
If you need passport services, please call the North Dade or South Dade Regional Library to schedule an appointment with a passport application acceptance agent. A separate appointment needs to be made for each applicant. For example, a family of five would need to schedule five appointments. Appointment scheduling is available daily, excluding Sundays.
Urgent Travel Plans
If you are traveling within 14 days or less, apply at a nearby Regional Passport Agency or Center . For more information, please call the National Passport Information Center (NPIC) at 1‑877‑487‑2778.
Photo Requirement
Passport photo service is currently not available at the North Dade and South Dade Regional Libraries. One official passport photo taken within the past six months is required. For additional photo requirements, visit the U.S. Department of State .
Customers are encouraged to visit the U.S. Department of State website for information and instructions prior to their appointment.

Passport Photo
One official passport photo taken within past six months. For additional photo requirements, visit the U.S. Department of State website .
Please Note
- New passport applicants must complete form DS‑11: Application for a U.S. Passport .
Forms can be obtained from the U.S. State Department website or at the North Dade and South Dade Regional Libraries. - Different forms are required for lost or stolen passports, name changes, renewals, etc.
For passport renewals , follow the instructions outlined on the DS‑82: U.S. Passport Renewal Application For Eligible Individuals . - Applications must be completed in black ink.
- Applications should not be signed until instructed by a passport agent.
- All passport applicants must be present.
- Applicants 15 and younger will need both parents present.

Payment Information
Two separate payments are required. One to the U.S. Department of State and the other to the Miami‑Dade Public Library System.
U.S. Department of State
The U.S. Department of State accepts only checks or money orders. Checks must be made payable to the U.S. Department of State. For applicable fees, view the U.S. Department of State Fee Calculator .
Miami‑Dade Public Library System
Customers must pay the Miami‑Dade Public Library System a $35 non‑refundable execution fee per application. The Miami‑Dade Public Library System accepts cash, cashier checks or money orders. Cashier checks or money orders must be made payable to the Miami‑Dade Public Library System.
Passport Service Options
The North Dade And south Dade Regional Libraries offer Routine and Expedited processing options. Processing times vary. Visit the U.S. Department of State website for up‑to‑date information .
National Passport Information Center (NPIC)
